  • Are there restrictions on which diagnoses are covered?
    Is diagnosis code Z71.3 (dietary counseling and surveillance) covered, or is a medical diagnosis required from my provider*?

  • Do I need a referral or prior authorization from a primary care provider?

  • How many visits are covered under my benefits?

  • Will I have a copay or coinsurance for nutrition services?
